2. Hazard communication standard, general industry (29 CFR 1910.1200) – 4,696 violations
3. Scaffolding, general requirements, construction (29 CFR 1926.451) – 3,814 violations
4. Respiratory protection, general industry (29 CFR 1910.134) – 2,371 violations
5. Ladders, construction (29 CFR 1926.1053) – 2,310 violations
6. Machines, general requirements, general industry (29 CFR 1910.212) – 2,097 violations
7. Powered industrial trucks, general industry (29 CFR 1910.178) – 1,993 violations
8. Electrical, wiring methods, components and equipment, general industry (29 CFR 1910.305) – 1,744 violations
9. Control of hazardous energy (lockout/tagout), general industry (29 CFR 1910.147) – 1,572 violations
10. Electrical systems design, general requirements, general industry (29 CFR 1910.303) – 1,332 violations
